Question:

How should iliac artery atherectomy be coded when performed with angioplasty and/or stenting during the same procedure?

Answer:

Atherectomy in the iliac arteries is reported with Category III code 0238T and could be separately assigned in addition to 37254–37262 if angioplasty and/or stenting is performed in addition to atherectomy. While code 0238T does not include vascular catheterization, codes 37254-37262 do. Therefore, if iliac atherectomy is performed in the same leg and through the same access as iliac angioplasty and/or stent placement, do not assign a catheterization code.
