Beginning October 1, 2025, CMS has established a new Transitional Pass-Through (TPT) payment for the WiSE™ CRT System under HCPCS code C1740, when billed in combination with CPT® code 0515T. This designation recognizes the system’s clinical innovation and ensures appropriate reimbursement during its early adoption phase. The WiSE™ CRT System represents a significant advancement in heart failure therapy. It is the world’s first and only leadless left ventricular endocardial pacing (LVEP) device designed for cardiac resynchronization therapy (CRT). Unlike traditional CRT systems, which rely on coronary sinus lead placement and can be limited by anatomy or venous access, the WiSE™ System delivers endocardial pacing from within the left ventricle, closely mirroring the heart’s natural electrical activation pattern. This endocardial approach is more physiologic and expands treatment options for patients who cannot receive conventional lead-based CRT—whether due to coronary venous anatomy, prior lead failures, phrenic nerve stimulation, or intolerance to transvenous CRT systems. By removing the need for a left ventricular lead, the WiSE™ System provides an alternative pathway to achieve meaningful resynchronization in complex heart failure patients.
