Question:

What is the correct coding approach when iliac atherectomy and angioplasty and/or stent placement are performed in the same leg through the same access site?

Answer:

Atherectomy in the iliac arteries is reported with Category III code 0238T and may be reported separately in addition to codes 37254–37262 when angioplasty and/or stenting is also performed. Although code 0238T does not include vascular catheterization, codes 37254–37262 do include catheterization. Therefore, when iliac atherectomy is performed in the same leg and through the same access site as iliac angioplasty and/or stent placement, a separate catheterization code should not be reported.
