Question:

What documentation practices does the ISMP recommend for specifying the mode and time interval of administration for higher-risk drugs?

Answer:

The Institute for Safe Medication Practices (ISMP) suggests that providers avoid generic reference to mode of administration (IV push, IVP, or bolus) for higher-risk drugs or drugs requiring administration lasting 1 minute or longer. When writing orders, the ISMP recommends that providers use more descriptive terms that include a reference to the time interval for the injection. (See the ISMP guidance on error-prone terminology in Appendix D.)
