Question:

An interventional cardiologist performs a PTCA in the LAD artery. The physician also performed angioplasty in the diagonal side branch of the patient’s LAD at the same session. How would we bill this in a hospital setting?

Answer:

Code 92920-LD and 92921-LD for a hospital setting. PTCA primary code (92920 ) is assigned for the PTCA performed in the LAD (major coronary artery). Because a branch vessel was treated in the same territory as the initial intervention, the PTCA of the diagonal branch is coded with add-on code 92921. Per CPT, add-on codes may only be assigned when treating additional vessels (no more than two) in the LAD, LCX, or RCA.
