Can we report 96409 for each different anti-neoplastic provided by an IV push?
This is not the correct way to bill under these circumstances. As the primary service, 96409 is reported for the first anti-neoplastic administered by IV push. Each different anti-neoplastic provided by IV push is billed using the add-on code 96411.
